How Medicare Works

Understanding the Parts of Medicare.

Medicare is a federal health insurance program administered by the Centers for Medicare and Medicaid Services (CMS). It is available to most Americans at age 65 and to certain individuals under 65 with qualifying disabilities. It does not function as a single plan — it is a system of parts, each covering a distinct category of healthcare.

Part A — Hospital Insurance

Medicare Part A covers inpatient hospital stays, limited care in a skilled nursing facility following a qualifying hospital stay, hospice care for terminal illness, and some home health care services. Most people do not pay a monthly premium for Part A if they or their spouse paid Medicare taxes for at least 10 years. However, there are deductibles and coinsurance for hospital stays that Part A does not cover — gaps that a Medigap Supplement plan or Medicare Advantage plan address.

Part B — Medical Insurance

Medicare Part B covers physician visits, outpatient services, preventive screenings, lab work, durable medical equipment, mental health services, and certain home health services. Part B requires a monthly premium — in 2025, the standard amount is $185 per month, though higher-income beneficiaries pay more through IRMAA surcharges. Part B has an annual deductible and covers 80% of approved services after the deductible, leaving the remaining 20% as your responsibility unless you have a Medigap Supplement plan.

Part C — Medicare Advantage

Medicare Advantage plans (Part C) are offered by private insurance carriers approved by Medicare. These plans bundle Part A and Part B coverage into a single plan, often with additional benefits not covered by Original Medicare, including dental, vision, hearing, fitness memberships, and over-the-counter allowances. Most Medicare Advantage plans also include Part D prescription drug coverage. Premiums are often low or $0 in addition to your Part B premium, but these plans typically require you to use a defined provider network and may require referrals for specialists.

Part D — Prescription Drug Coverage

Medicare Part D provides outpatient prescription drug coverage. Part D is offered through private insurance carriers as either a standalone plan (paired with Original Medicare) or as part of a Medicare Advantage plan. Each Part D plan has its own formulary — the list of covered drugs — and its own premium, deductible, and cost-sharing structure. Choosing the wrong Part D plan or failing to enroll when first eligible triggers a lifetime late enrollment penalty of 1% per month of the national base beneficiary premium for every uncovered month.
